​Scheduling your first appointment can be intimidating. Below are some instructions to help you schedule your first appointment and a description of what to expect and what forms and paperwork need to be completed in order to make the first visit a success

1. If you have been prescribed medication prior to seeing our professionals, or you have received care from a different practitioner, we request you authorize having your records sent to our office. To do so, complete the release of information form below and email to [email protected].

2. Once completed, please call us at 402-881-0678 to schedule your first appointment. Once you have scheduled your appointment you are to complete information through the onpatient portal. The onpatient portal is our patient access system that allows your to directly interact with the provider and clinic staff in a secure HIPPA compliant format.

​​3. Bring a copy of your insurance card, a photo ID (driver's license, state ID, or military ID), a list of medications you are currently taking or have taken in the past (bottles are fine). Your first appointment should take about one-and-a-half hours.

4. Continue to follow up with appointments, and call our office with any questions or concerns.
